She was the second of four children born to Richard Hilton and Kathy Richards. Her sister and brothers are Paris Hilton, Barron Hilton and Conrad Hilton. On the maternal side of her family, Nicky Hilton is a niece of two popular child stars of the 1970s, Kim Richards and Kyle Richards, who appeared in the motion picture Escape to Witch Mountain and TV shows like Nanny and the Professor, Little House on the Prairie, and later, ER. Her paternal grandparents are Barron Hilton and Marilyn Hawley. She shares her nickname with her grandfather's brother Conrad "Nicky" Hilton, who died 14 years before she was born.

Nicky Hilton moved between several homes in her youth, including a suite in the Waldorf-Astoria Hotel in Manhattan, Beverly Hills, and The Hamptons on Long Island, New York. Currently, her parents own a $10,000,000 USD mansion in Bel Air, California, a $6,300,000 USD estate in The Hamptons, as well as a $3,000,000 USD home in the Hills of Hollywood in which Nicky and her sister, Paris, live to give them privacy and easy access to the L.A. clubs they love to frequent.

Hilton and sister Paris are known for their jet-set lifestyles, though Nicky has kept a lower profile. In 2003, Nicky was offered a role in a reality show with her sister, Paris Hilton, The Simple Life. On August 15, 2004, Nicky Hilton married Todd Andrew Meister, a Manhattan money manager, at a Las Vegas wedding chapel with Bijou Phillips and Tara Reid as bridesmaids, although the marriage was dissolved shortly afterwards.

She dated Ian Somerhalder (Lost) in 2003 and as of January 2006 was dating Entourage star Kevin Connolly.

In 2005, Hilton, along with Kimberly Stewart, was the face of Australian underwear line, Antz Pantz. Stewart remains contracted, however Hilton has since been replaced by Australian up-and-coming model Megan Maitland.
